Tesamorelin is a synthetic analog of growth hormone–releasing hormone (GHRH), specifically engineered to stimulate the pituitary gland to increase endogenous growth hormone (GH) secretion. Structurally modified from natural GHRH (1–44), tesamorelin is stabilized to resist rapid enzymatic degradation, resulting in a longer half-life and more sustained biological activity in research settings.

Mechanistically, tesamorelin binds to GHRH receptors located on somatotroph cells in the anterior pituitary. Upon receptor activation, intracellular signaling pathways (primarily via cAMP) are triggered, promoting pulsatile growth hormone release. Elevated GH subsequently stimulates hepatic production of insulin-like growth factor 1 (IGF-1), a downstream mediator responsible for many anabolic and metabolic effects observed in research models.
